The following are times when you should NOT send your child to school:
- Vomiting or diarrhea in the past 24 hours
- Fever of 100° or higher. Should be fever-free, without medication for 24 hours before returning.
- When he or she has a severe cold and/or bad runny nose
- Undiagnosed rash (healthcare provider will need to diagnose)
- Persistent cough
- If on an antibiotic, should be home for 24 hours before returning
- When your child "looks sick" or has other symptoms that common sense tells you to keep your child home
- If your child becomes sick at school, you will be called to pick them up
Health Forms That Must be Completed Yearly

- Parent Medication Permission Form
- Must be completed for every medication taken at school.
- Athletic Physical Examination Form
- This form must be filled out yearly if your 7th-12th grade child plans to participate in athletics and/or cheer-dance. Forms are due the first day of practice and expire after 1 year.
- Concussion Waiver
- This form must be completed by each 7th-12th-grade student & parent before the first day of practice and expires after 1 year.

- 3-Year-Old Preschool Entry
- 3 Year Old Physical Form
- This form can be completed by your child's doctor/health care provider before preschool entry. Also, please provide your child's updated immunization record.
- 4-Year-Old Little Paws Preschool Entry
- SOS Physical Form
- This form can be completed by your child's doctor/health care provider before preschool entry. Also, please provide your child's updated immunization record.
- Transitional Kindergarten/Kindergarten Entry
- SOS Physical form
- Dental Screening
- Vision Screening
- Forms can be completed by your child's doctor/health care provider before TK/Kindergarten entry. A blood lead level is required and can be done at your child's physical appointment. Also, please provide your child's updated immunization record.
- Please note that if your student was in TK and all 3 forms were already filled out, they do not need to be filled out again.
- 3rd-Grade Entry
- Vision Screening
- Form can be completed by your child's eye doctor.
- 9th-Grade Entry
- Dental Screening
- Form can be completed by your child's dentist or registered dental hygienist.
Immunization Requirements by Grade
Every student entering school must provide the school with a copy of updated immunizations. This is a requirement before the student may start school. Additional immunizations required are listed below by grade. If your child does not receive vaccines, please see the Immunization Exemption section below.
- Before Kindergarten Entry
- 5 DTaP doses with 1 dose after age 4
- 4 Polio doses with 1 doses after age 4
- 2 doses MMR
- 2 doses Varicella after 12 months of age OR proof of having the disease
- 3 doses of Hepatitis B
- Before 1st-6th-Grade Entry
- All childhood immunizations are required (see immunizations listed under Kindergarten entry)
- Before 7th-Grade Entry
- All childhood immunizations are required in addition to the following
- Tdap (tetanus, diphtheria, and pertussis)
- 1 dose Meningitis after age 10
- Before 9th-11th-Grade Entry
- All childhood immunizations and Tdap and Meningitis vaccines are required
- Before 12th-Grade Entry
- All childhood immunizations, Tdap and Meningitis #1 are required in addition to the following:
- 1 dose Meningitis after age 16 OR 2 doses if received 1 dose before age 16

- If your child has a medical or religious exemption and does not receive vaccines, one of the following forms must be on file at school.
